There are two expenses you will incur. First there is a setup fee that is a one time non-refundable fee. This fee allows us to setup up your website account and customize appropriate images specific to you or your agency and to setup your personal profile database. If you are subscribing to our TOUCH product then there is a one time setup fee as well. The setup fee varies depending on the plan option you select. This one time fee (based on non-interrupted service) which includes all future modifications the agent may require including changes to, text, images, links, product icons, providers, contact info, and navigation for the website option. Should changes be required beyond this scope a nominal fee may be applicable.
The second fee is a monthly subscription fee based on the number of carriers you wish to display (please note not all carriers are supported). These fees range from $59.00 per month up to $169.00 per month depending on plan selection. The monthly subscription fee is for your license to participate in and maintenance of the platform which may include your web site. Agents may add or remove carriers as needed without any additional service charge and the applicable monthly rate will be applied for the next billing cycle.
Unless you signed up under a promotional offer there is no contract requirement. Beyond the non-refundable Setup Fee, you can terminate your service at any point during a billing cycle. We request that you provide us with at least 10 day notice of your wish to terminate service. The request to terminate service may be sent to marketing@indiviualhealth.com. |
