**Embedded in the PPO Plan 5000 is the Critical Condition Benefits. Members who have a first incident of severe heart attack, severe stroke or life-threatening cancer become eligible for this benefit. There are restrictions that apply. Proceeds paid to the subscriber from the critical condition benefit under this policy may be used for non-medical care expenses.
