Basic PPO 1000/2500 - The Basic PPO Plans offer solid protection at affordable prices and let you choose either a $1,000 or $2,500 annual medical deductible. These plans offer basic coverage for hospitalization and emergency services. Once you meet your annual out-of-pocket maximums, doctors' office visits are also covered. These plans do not include maternity coverage or prescription drug benefits.

PPO Share Plans - The PPO Share Plans are structured to provide a more comprehensive list of benefits while providing the most flexibility in your choice of doctors and hospitals. The difference is in the deductibles, coinsurance amounts and annual out-of-pocket maximums. Blue Cross offers a variety of PPO Share Plans so that you can more precisely choose the best pricing options for you.

PPO Saver Plans - The PPO Saver Plans is a mid-range cost option providing valuable benefits for hospitalization, emergency services and prescription drugs. And you receive more benefits after you meet annual deductible/annual out-of-pocket maximum requirements. The no-frills features help keep you monthly premiums low, and you'll carry peace of mind with you wherever you go.

The RightPlan PPO 40 and the 3500 Deductible PPO plans are tailor-made for young adults, empty nesters or early retirees. They offer low-cost, comprehensive coverage without the added-on benefits that many people know they will never use - such as maternity. With the 3500 Deductible PPO, just meet your annual deductible and you'll then pay $0 for most in-network covered services.

With the RightPlan, you have the option to choose what level of prescription drug coverage is best for you. These plans were designed specially to meet the needs of those individuals who are looking for a lower cost plan with no deductible and a choice of prescription options. If you are a young, active healthy adult, an over age dependent, self-employed single, empty nester or early retiree, then the RightPlan PPO 40 might be a plan worth considering.

HMO Plans - Blue Cross of California offers three HMO Plans to choose from. The Select HMO, HMO Saver and Individual HMO Plans. If your doctor dosen't participate in the Select HMO Network then consider the HMO Saver or Individual HMO plans. Both the HMO Saver and the Individual HMO offer rich benefits. Choose the HMO Saver if you want lower monthly premiums and don't mind meeting a $1,500 deductible. Choose the Individual HMO if you want immediate, no-deductible benefits.

Lumenos Plans offer three plan series to choose from. The Lumenos HSA Compatible Plans provide you the ability to save tax-free dollars in an account. Lumenos HIA Plan provides you the ability to use funds allocated to an account set up for your to help pay for your medical care and presecriptions. The Lumenos HIA Plus Plans allow you to use funds allocated to an account set up for you to pay for your medical care and prescriptions, plus you can earn additional dollars in your account with Health Rewards incentives. All unused HIA funds roll over to the next year - there is no limit on the amount you carry over.
