Dental Blue PPO Dental Plans from Anthem Blue Cross
Now you have more choices for your Dental coverage. Anthem Blue Cross and Anthem BC Life & Health now offers two plan designs. Anthem Dental Blue PPO and Anthem Dental SelectHMOSM plans.
The Dental Blue PPO Plans from Anthem Blue Cross offer you an affordable PPO solution to meet your dental needs - all designed to give you the power to choose. The Dental Blue PPO Dental Blue 100 Network of providers allows access to more than 25,000 participating providers in California and more than 71,000 Dental Blue providers nationwide.
Two PPO Plans to choose from...
Dental Blue Basic - Offers essential coverage at a value price:
- Diagnostic and preventive coverage for services like cleanings, exams and X-rays
- Basic coverage for fillings
- Benefits for stainless steel crowns and pulpotomies (removing infected portions of pulp tissue) on primary (baby) teeth
- An annual maximum of $500

There is a six-month waiting period for restorative services. Anthem pays out-of-network services based on the Dental Blue 100 fee schedule.
Dental Blue Enhanced - offers more coverage
- Diagnostic and preventive services
- Benefits for basic services
- Major services like crowns, periodontal procedures, oral surgery, and root canals
- Orthodontic coverage for children after a 12-month waiting period, with a separate lifetime maximum of $1,000 ($500 per year)
- An annual maximum of $1,250
- There is a six-month waiting period for basic services (such as fillings) and a 12 month waiting period for major services.

With Anthem Blue Cross dental plans, members get more for their money. For example, members pay Anthem's negotiated rates for covered services from in-network dentists:
- During waiting periods
- When they've exceeded their annual maximums
- When they've exceeded contract limits like coinsurance and deductibles, and frequency ,imitations. (For example, if a plan covers two cleanings each year and a member goes for a third cleaning, the member would pay Anthem's negotiated rate for that third cleanings)

You might pay more when you visit an out-of-network dentist...
While your dental plan lets you choose your dentist, you may end up paying more for a service if you visit an out-of-network dentist. Here's why: In-network dentists have agreed to payment rates for various services and cannot charge you more. On the other hand, out-of-network dentists don't have a contract with Anthem and are able to bill you for the difference between the total amount Anthem allows to be paid for a service - called the "maximum allowed amount" - an the amount they usually charge for a service. When they bill you for this difference, it's called "balance billing"
Who’s Eligible for Blue Cross Dental Plans
You and your dependents must be California residents. If you enroll in one of the Anthem Blue Cross Dental SelectHMO plans, you and your dependents must select the same participating dental office. Dental providers and locations are different for the Dental Blue plan. Eligible dependents include:
- Your lawful spouse
- Any unmarried child under age 19 of you or your enrolled spouse
- Any unmarried child, age 19 to 23, of you or your enrolled spouse, who qualifies as a dependent for federal income tax purposes
- A child of you or your enrolled spouse, who continues to be both incapable of self- support, due to continuing mental retardation or physical handicap, and who is at least one-half dependent on you or your spouse for support.

HOW TO APPLY:
There are three options to enroll in one of the Anthem Dental Plans. The quickest and easiest is to complete the online enrollment form. If you are ready to apply and you want to complete the process online then click the orange button below. You must be 64 years of age or less to use the online application process.
The second option is to download and complete the enrollment form and fax it to us. To choose this option simply download the appropriate enrollment form below. This includes those who are age 65 or older:
