Prescription Drug Coverage
SnartSense Plus PPO includes coverage for generic and select brand name and specialty drugs. For an additional cost, you can upgrade the SmartSense Plus PPO prescription benefit to extend the coverage for brand name and specialty drugs. You will receive the hightest level of benefits by asking your phycisian to prescribe a genric drug whenever possible. If you choose to purchase a brand name drug on the formulary, when a generic drug is available, you will be responsible for the difference in the cost between brand and generic, plus your copayment or coinsurance.
How to Customize your SmartSense Plus Plan
With SmartSense Plus PPO, you have some choice and flexibility to change the plan to better meet your needs. SmartSense Plus offers a choice of:
Deductible: SmartSense Plus PPO deductibles range from $750 to $20,000. You can usually lower your premium by choosing a higher deductible. Simply choose the deductible and premium combination that works best for you.
Prescription Drug Benefit:You can customize your plan by selecting the Optional Enhanced Prescription Drug coverage, as described on your Benefit Guide
 Other Optional Coverage: You can add more protection for you and your family by purchasing optional dental or life insurance. The e Dental PPO plan includes coverage for the basics, plus certain services like crowns, root canals and dentures. If you need a dental plan that offers important preventive services and a broad range of benefits, Blue Dental PPO could be the right plan for you. Apply for BlueChoice as a stand alone plan or add it to your medical at the time of application.
Add term life insurance
