How Premier plans work
Because Premier plans are PPO (preferred provider organization) health plans, each plan:
- Has an annual deductible. This is the amount you have to pay out of pocket each year before your plan starts paying. (Deductibles don't apply to your network doctor visits)
- Doctor Office Visits. Pay a $30 Copay for primary care physician and $40 Copay for specilist
- Starts paying a portion of your covered medical expenses once your annual deductible is met. Your share costs for covered services is called your coinsurance.
- Saves you more money when you use a doctor in the PPO Network. For a list of network doctors, go to anthem.com, access your state's homepage and click on "Find a Doctor".

Built-in prescription drug coverage
The high cost of today's prescription drugs can be financial drain on your family. Premier includes prescription drug coverage to help you manage those costs. Premier offers broad prescription drug coverage before the deductilbe, including benefits for generic, brand name and specialty drugs.
Genric drugs: You will receive the highest level of benefits by asking your physician to prescribe a generic drug whenever possiblw. You choose to purchase a brand name drug when a generic drug is available, you will be responsible for the difference in the cost between brand and generic, plus your copayment or coinsurance.
Network Discounts = Real Money...
With Anthem Blue Cross and Blue Shield you have access to one of the largest provider networks in the state of Ohio. These network (or participating) providers have agreed to accept lower costs for their covered services to Anthem members - similar to volume discounts. These negotiated costs help reduce the overall cost of covered medical services, including your share of those costs.
This is true whether you are paying the entire cost for covered services (such as while you are meeting your deductible), or whether Anthem is sharing the cost. With nearly 7,500 doctors and over 75 hospitals, chances are your provider already participates. Just visit a network provider to take advantage of the savings. With Anthem PPO plans, you can always choose to receive services outside the network, but your share of the cost will be greater. |

Preventive Care Benefits
Now all of the Individual Plans from Anthem Blue Cross Blue Shield of Virginia include 100% coverage for Preventive Services when you utilize a network provider. This follows the health care reform law and state regulations. When you get these services from providers in the network, you don't have to worry about paying anything out of your pocket for covered preventive care such as screenings, immunizations and exams. You may have to pay part of the costs if you use a provider outside the network.
Preventive versus Diagnostic Care
Preventive care is precautionary. Diagnostic care us used to find the cause of existing symptoms. For example, if your doctor suggest you have a colonoscopy because of your age, that's preventitive care. Optional Coverages
Maternity

An optional maternity coverage is available with certain plans to help cover pregnancy and childbirth related medicare care for mother and infant. There are specific limitations and exclusions for this coverage, including a waiting period in most cases before conception can occur.
Dental Coverage

Anthem offers three packages to complement your Medicare Supplement plan - and help you reach for better health. - Support services and tools to help you maintain good overall health and well-being
And best of all, these packages are available for a monthly plan premium ranging from $18 to $52. The benefits in each package will roll up to one overall premium, and you will receive one ID card for all of these services.
Interested in Dental Only Coverage?
Anthem Blue Cross Blue Shield of Virginia offers a stand alone Dental Only option for seniors. The Anthem Extras Package contains the option for the Premium Plus package which part of this package is their PPO Dental Plan. As long as you have Medicare A & B you can choose to enroll in the Anthem Premium Plus Dental Plan on a Stand-Alone basis. Premium Plus lets you choose any dentist whether or not that dentist is part of their network. But you may end up paying more for a service if you visit an out-of-network dentist.
