The Anthem Blue Short Term Plan is designed to protect you for one to siz months. No matter how healthy you are or how well you take care of yourself, unexpected health issues can arise. With Anthem Blue Short Term plan, you get choices. Decide for yourself what fits your needs. When do you want coverage to start? How long will you need coverage - one, two, three, four, five, or six months? Select the deductible that's right for you, from as low as $250 all the way up to $2,500. And with that choice, you can influence what your coverage will cost.
What's Covered?
Blue Short Term offers the same level of reliable healthcare coverage you'd expect from the company that's been protecting people for more than 60 years. If you still need temporary insurance when your first Blue Short Term plan expires and you are still able to answer "NO" to the questions under Part E of the application, you may reapply as often as needed (within a maximum term of 360 days) by completing a new application.

Covered Services:
 Office Visits- Prescription drugs - up to $500
- Diagnostic services (lab and x-ray)
- Inpatient hospital and outpatient services
- Emergency room and urgent care
- Ambulance - up to 40 visits
- Hospice
- Human organ and tisse transplant - up to $1,000,000
- Durable medical equipment - pays 50% of the covered services after deductible
Once your deductible has been reached, Blue Short Term plan pays 80% of covered services. You pay the remaining 20% until your total out-of-pocket expense for covered services is met. Once that limit is reached, the plan pays 100% for most covered services, up to the $1,000,000 maximum. |

Assurant Health Short Term Plans for Ohio Residents
 There are times in ones life where the need for Short Term Coverage provides a perfect solution for those in between times. It is important to note that Short Term Plans are not designed to replace permanent coverage. There are two types of short term plans now available from Assurant Health to Ohio Residents.
Single Pay Short Term Plans: (Assurant Health)

Choose the duration of coverage from 30 - 185 days

Choose deductible amount $250, $500, $1,000, $1,500 or $2,000

Prescription drug coverage (after your deductible has been met)

After you meet your deductible, the plan pays 80 percent of allowable amount of the first $5,000 of covered services, and 100 percent of allowable amount for covered services in excess of $5,000.

Have the security of a high, $2 million maximum benfit per policy term
Monthly Payment Plan: (Assurant Health Twelve Month Plan)

Continuous coverage from month to month up to 12 months, with monthly billing

Coverage may begin the day after your application is submitted (when approved), or a future effective date specified within 45 days.
