How to Customize your Lumenos HSA Plus Plan
Chose your deductible: You can usually lower your premium by choosing a higher deductlbe. Simply choose the deductible and premium combination that works best for you. Remember, any covered member can contribute to some or all of the policy deductible and out-of-pocket maximum, whether the policy covers one member or a whole houseful
Use you Health Savings Account the way you want: Your HSA, if you choose to open one, if funded by you. So, it is yours to use for qualified health care expenses covered by the plan, or those not covered at all, like contact lenses. Your HSA is also yours to keep if you ever leave the plan; you won't lose those dollars if they're not used. If fact' the carryover from year to year can help you save for future financial needs

You should know:
- Maternity benefits are not available with this plan
- Your Lumenos HSA Plus plan has a policy level deductible and out of pocket maximum. Once and combination of covered members on the policy meet these amounts, the plan pays 100% of covered expenses
- While Lumenos HSA Plus is compatible with a Health Savings Account, your health care plan works with or without it. You may set up the HSA now, later, or not at all. It's your choice

Prescription Drug Coverage
Lumenos HSA Plus not only puts you in charge of your health care dollars, it can help you use those dollars for generic and brand name prescription drugs in the way that best suits you.
Once your deductible is met, there is a coinsurance, if applicable, for covered prescription drugs. But even while you are meeting your deductible, you benefit from lower negotiated rates on prescription drugs at network pharmacies nationwide. There's no need to have a different deductible or copayment for prescriptions; it all works as one.
Adn since you decide how to spend it, your Health Savings Account dollars can be used to pay for prescription drugs - either you are meeting your deductible, or afterward for those drugs not covered, like most over-the-counter medications. |
Medicare Supplement Plans for Indiana
A Medicare Supplement Plan is a health benefit coverage plan that complements (or supplements) your regular Medicare coverage. Although Medicare pays for many health care services and supplies, it doesn't pay for all of them. A Medicare Supplement Plan will help pay for those costs which could include deductibles, coinsurance and copayments when visiting your doctor. An Anthem Medicare Supplement Plan is also a good idea if you prefer to keep your own doctor and hospital and choose your own specialist and other health care provider.
Who is eligible?
You are eligible for a Medicare Supplement Plan if you are enrolled in Medicare Parts A and B and if your are age 65 or older or, in some states, under 65 with a disability and/or end stage renal disease (plan offerings and eligibility vary by state). Preventive Care Benefits
Now all of the Individual Plans from Anthem Blue Cross Blue Shield of Virginia include 100% coverage for Preventive Services when you utilize a network provider. This follows the health care reform law and state regulations. When you get these services from providers in the network, you don't have to worry about paying anything out of your pocket for covered preventive care such as screenings, immunizations and exams. You may have to pay part of the costs if you use a provider outside the network.
Preventive versus Diagnostic Care
Preventive care is precautionary. Diagnostic care us used to find the cause of existing symptoms. For example, if your doctor suggest you have a colonoscopy because of your age, that's preventitive care. But, if you doctor suggests a colonoscopy to see what's causing your symptoms. that's diagnostic care and you may need to pay part of the cost. Deductible Credit Program Available in Indiana
The Deductible Credit Program is a program for Individual customers that automatically puts money into a Special Incentive Account (Account) when you don’t meet your annual deductible, and grows that account to use for future medical costs or whatever you want. If you are a healthy person who rarely meets your deductible or a person who likes having a high deductible in order to keep your premiums lower than this may be of interest to you.
First in order to qualify for one of these accounts you must be on a plan that has a deductible of at least $2,500 or higher or $3,000 or higher for the Lumenos HSA plan. Once you are eligible your account will be set up automatically. Each year where your medical claims are less than your deductible Anthem will credit your Account with an amount that is equal to 20% of your deductible ($3,000 Deductible x 20% = $600).
Once you accumulate credits in your Account, Anthem will automatically send you a special check from your Account the first year you have claims greater than 80% of your deductible. You can use the check payment to lower your out-of-pocket expenses or on whatever you deem necessary. Once this happens Anthem will reset your account back to zero the following year. Then your credits will start all over again.
This new program can help soften the expenses you may incur over your lifetime thus helping reduce the impact of having a high deductible plan.
